Customer Issue Directions

  1. Print the Customer Issue Form and have customer complete form. Make sure customer receives a copy. Fax or scan and email our copy to Bud. New!!! A POA will now print with the claims form.  Have the customer sign Section 9.  Claims Administrator will fill out the rest.
  2. Enter a summary of the information on the Customer Issue Log Entry Form
